With respect to a diary or journal,  I use a composition/memo book you can buy anywhere, 7 1/2 x 9 3/4.  I start from the back and create a calander with the year that I had my first seizure. I then create a grid with 1 in x 1in squares, one for each month. One year is one line.  In each month, I enter when I had a seizure the type as well as my blood test results.  Granted I only have one or two a month, if lucky and have my blood level checked every few months so this is ample for me.  Larger squares can be arranged as you wish.  The idea is as time goes on, you turn pages towards the front.  You could have one page per year.    This arrangement allows me use a highlighter to highlight the different types of seizures and than I can see any patterns. From the front of the book, I enter my ongoing dialog of thoughts.  The dates link back to the calendar and I can add as much or as little as I wish there.  When I have my annual physical
or visit my neurologist,  I have a list of questions I collect in the front between appointments and I add comments from each appointment.  I'm fortunate my neurologist has an email address he shares with me that you can ask him a question or work with him as needed.  Most of this is generic in nature, I need a prescription, and he mails me one.  The issue with personnel information I have to be careful.  

    In any case, this is how I set up my diary and it works well providing a summary in the back, i.e. big picture, and details in the front like a regular diary.  Hope it helps others.  Thanks for your support and I am smiling more now that I've joined the group.
