Erin,

First it depends on what type of surgery. The implantation of a VNS or
brain surgery. The VNS is decided between the patient and the Dr. The
decision to have brain surgery is s totally different thing.

The process is very complicated and involves a lot of testing. An
epileptologist first has to declare that you have medically intractable
seizures, those that are not controlled by medication alone. At that
point a patient is referred for many tests to determine if they are even
a candidate. These include, but are not limited to, PET scan, MEG
scan, 3T MRI, a neuro psych evalutation, a WADA test, an inpatient 24
hour monitored EEG.

Once all of that is done a surgical board gets together and reviews all
of the information. This board consists of several physicians,
including the epileptologist, a neurosurgeon, the neuro psychologist,
and a couple others. THey make the final decision if someone is a
candidate for surgery.
